Fertility expert dispels myth of boxers over briefs for higher sperm count
There are 8 things you can do to keep your sperm in peak condition - and wearing boxer shorts is not one of them. Don't stress about your underwear choice, boys - briefs or boxers make no difference to sperm count, a fertility expert claims.
A Queensland fertility expert confirms there is no concrete evidence to support the theory that "shooting the breeze'' in boxers, not briefs, boosts sperm quality. Dr. David Molloy, Medical Director of Queensland Fertility Group, said male factor infertility affects about half of all infertile couples. "It is important to understand how the male reproductive system works, and dispel some common myths and misconceptions," Dr Molloy said. "One I hear all the time is that tight underpants are bad for a man's sperm count. There is no concrete evidence that wearing underpants instead of boxer shorts causes poor quality sperm. This common fertility myth is based on the fact that sperm do not like high temperatures. But basically maximising sperm quality essentially comes down to common sense - healthy men produce healthy sperm,'' he said. The fertility doctor has some realistic recommendations for men keen to be dads:
1. Sperm do not like high temperatures and it's best to avoid having regular hot baths and saunas.

2. If you want champion swimmers you should keep weight stable, as being overweight can disrupt sex hormone production.
3. To ensure peak mobility and avoid DNA damage, skip cigarettes, drugs, steroids, sexual lubricants and limit alcohol intake.
4. If you have a desk job, get up and move around regularly.
5. Have frequent sex to ensure sperm is not in prolonged storage in the epididymis.
6. Eat antioxidant-rich food such as berries, pumpkin seeds, colourful vegetables and green tea, and consider a multivitamin supplement with high levels of antioxidants such as Vitamins C and E. Antioxidants protect sperm from free radical damage, which can occur during storage in the testes.
7. Avoid naturopathic products that have components with an estrogenic action as they suppress sperm production.
8. Know your medications. Treatments for things like cancer, inflammatory bowel disorders, blood pressure medications and immune system suppressants can reduce sperm quality and damage sperm production.

Male factor infertility accounts for approximately 50% of all infertility cases among couples seeking treatment in the US. Common causes include low sperm count, poor sperm motility, abnormal sperm morphology, and issues with sperm production or function. According to the American Society for Reproductive Medicine (ASRM), a semen analysis is typically the first diagnostic test recommended when evaluating male infertility.
Does temperature really affect sperm production and quality?
Yes, excessive heat can negatively impact sperm production since the testes require a temperature slightly lower than normal body temperature to function optimally. Men should avoid prolonged exposure to hot baths, saunas, and tight-fitting clothing that traps heat in the genital area. However, regular underwear choice (boxers vs. briefs) has not been proven to significantly affect sperm count based on available scientific evidence.
What lifestyle changes can improve male fertility?
Key lifestyle modifications include maintaining a healthy weight, avoiding smoking and recreational drugs, limiting alcohol consumption, eating an antioxidant-rich diet with foods like berries and vegetables, and staying physically active. Regular sexual activity and avoiding prolonged heat exposure are also recommended. Additionally, men should review their medications with their healthcare provider, as certain treatments for cancer, blood pressure, and inflammatory conditions can impact sperm quality.
Are fertility supplements for men covered by insurance?
Coverage for male fertility supplements varies significantly depending on your insurance plan and whether they are prescribed by a doctor versus purchased over-the-counter. Most commercial insurance plans do not cover over-the-counter supplements, though some FDA-regulated products may be covered if deemed medically necessary and prescribed by a fertility specialist. It's best to check with your specific insurance provider or discuss supplement recommendations with a fertility clinic affiliated with your health plan.
Which medications can negatively impact male sperm quality?
Several medication classes can reduce sperm quality, including certain cancer treatments (chemotherapy), immunosuppressants, some blood pressure medications, and treatments for inflammatory bowel diseases. Anabolic steroids and other performance-enhancing drugs also significantly impair sperm production. Men should always inform their fertility specialist or urologist about all medications they're taking, as alternatives may be available that pose less risk to sperm health.
How long does it take to improve sperm quality through lifestyle changes?
It typically takes about 3 months (one full cycle of sperm production) to see improvements in sperm quality after making lifestyle changes, since sperm take approximately 74 days to mature. Men committed to diet improvements, weight loss, smoking cessation, and other healthy habits should plan for at least a 3-month timeline before undergoing repeat semen analysis. The American Society for Reproductive Medicine recommends consistent lifestyle modifications during this period for optimal results.
What antioxidants are most important for sperm health?
Vitamins C and E are among the most well-researched antioxidants for protecting sperm from free radical damage during storage in the testes. CoQ10, zinc, and L-carnitine are also clinically studied nutrients shown to support sperm health and motility. A balanced approach combining antioxidant-rich foods like berries, pumpkin seeds, and colorful vegetables with a quality multivitamin can help maximize sperm quality without excessive supplementation.
